We are now providing IUD insertion/removal, IUD checks and Nexplanon to patients of surgeries within the North Cardiff Cluster as well as our own patients, and accept self-referrals.

In the first instance, please check with your surgery if they are within the North Cardiff Cluster. Once confirmed, please request a copy of your medical history summary from your own surgery and attach this to the referral form below. Once completed please take to reception at North Cardiff Medical Centre.

Please do not complete a self-referral if you are a patient at North Cardiff Medical Centre. You will need to contact the surgery for an appointment with Dr Sherwood.
